The Kaiser Permanente Medicare Advantage plans for individuals include Silver & Fit^{® *} - a gym or at-home fitness program—for no additional cost.
For an affordable monthly fee, you can purchase the Advantage Plus† package, which offers extra coverage for comprehensive dental, hearing aids, eyeglasses and contact lenses.

Keep the health care team you know and trust.

Access to a wide range of specialists and your choice of available quality Kaiser Permanente doctors—and you can switch to another available Kaiser Permanente doctor if your needs change.

The Kaiser Permanente physicians you already know and trust will continue to coordinate and provide your health care at our medical centers.
Your medical record number and your electronic medical record won’t change.
You’ll keep your same kp.org account and password.
And your benefits will still include preventive dental, routine eye care and can include prescription drug coverage.

Physician Options	To receive benefits of the Kaiser Permanente Medicare Plus (Cost) plan, you must use Mid-Atlantic Permanente Medical Group physicians and affiliates. You may go to any other provider who accepts Medicare and receive Original Medicare benefits.	To receive benefits of the Kaiser Permanente Medicare Advantage plan, you must use Mid-Atlantic Permanente Medical Group physicians and affiliates. No benefits for other providers. NOTE: If you are already scheduled for treatment after January 1, 2019 with a provider who is not part of the Kaiser Permanente Medicare Advantage network, please download and fill out the Continuity of Care form in Helpful Documents and contact Member Services at 1-888-777-5536 (TTY 711), Monday through Friday, 8 a.m. to 8 p.m. so we can determine if you are eligible to temporarily continue care with that provider.

In the District of Columbia, Kaiser Permanente is an HMO plan with a Medicare contract. In Maryland, Kaiser Permanente is a Cost plan and an HMO plan with a Medicare contract. In Virginia, Kaiser Permanente is a Cost plan with a Medicare contract. Enrollment in Kaiser Permanente depends on contract renewal. You must reside in the Kaiser Permanente Medicare health plan service area in which you enroll.
